Tolcapone Dosage

When treating Parkinson's disease, the recommended starting tolcapone dosage is 100 mg three times daily. If absolutely necessary, your healthcare provider can increase your dose up to 200 mg three times daily. The exact tolcapone dosage your healthcare provider recommends will vary, based on several factors, such as how you respond to the drug, other medical conditions you have, and other medications you are taking.

Tolcapone Dosage: An Introduction

The dose of tolcapone (Tasmar®) your healthcare provider recommends will vary, depending on a number of factors, including:
 
  • How you respond to tolcapone (especially if you develop dangerous side effects)
  • Other medications you are taking
  • Other medical conditions you may have.
     
As is always the case, do not adjust your dose unless your healthcare provider specifically instructs you to do so.
 

Tolcapone Dosage for Parkinson's Disease

Tolcapone is always used in combination with carbidopa-levodopa products (Sinemet®, Sinemet CR, or Parcopa®). The recommended tolcapone dosage for Parkinson's disease treatment is tolcapone 100 mg three times daily. Your healthcare provider may increase your dose up to tolcapone 200 mg three times daily, but only if absolutely necessary, as the higher dose increases the risk of liver problems. If you do not experience any noticeable benefit from tolcapone within three weeks, let your healthcare provider know, as the drug should only be taken if it is beneficial.
 
The first tolcapone dose of the day should be taken with your first carbidopa-levodopa dose of the day. The second dose of the day should be given about six hours later, with the third dose taken about six hours after the second dose.
 
Do not suddenly stop taking tolcapone, as it may cause a serious group of side effects known as neuroleptic malignant syndrome (NMS). Symptoms of NMS include:
 
  • A high fever
  • Stiff muscles
  • Confusion
  • Irregular pulse or blood pressure
  • A fast heart rate (tachycardia)
  • Sweating
  • Irregular heart rhythms (arrhythmias).
